AY 2020-2021 Enrolment Information Bulletin for Freshmen

What are the documents required for enrolment?

  1. Original copy and two (2) photocopies of Form 138 (Senior HS Card)
  2. Original copy and two (2) photocopies of Certificate of Good Moral Character (from Guidance Counsellor or School Principal)
  3. Authenticated copy and one (1) photocopy of Birth Certificate from PSA
  4. Two (2) copies of 2x2 bust size picture which should be recent and unretouched
  5. Photocopy of your parent’s latest income tax return or certification from BIR

 

Where do I mail my documents?

Please place all documents inside a long size folder, enclosed in a long size brown envelope, and send through courier service to:

When to submit?

Please send the requirements to the Office of Admissions and Scholarship Administration (OASA) on or before June 30, 2020.  If the OASA will not receive the documents by July 10, 2020, your name will be deleted from the list of enrolees.

When is the start of enrolment?

Upon receipt of your documents, your program adviser will automatically enrol you to the program.  The program adviser will email your Certificate of Registration (COR).
